The incident occurred when Adade appeared to push Peral as he tried to retrieve the ball for a quick restart.
Kotoko have already announced that disciplinary action has been taken against Peral, including a fine.
The GFA has now formally charged the technical team member with breaching Article 12(1)(h) of the GFA Disciplinary Code.
Peral has until Wednesday, November 4, to respond to the charge.
Kotoko drew with Medeama and are now without a win in their past two matches. They sit ninth in the league table with 13 points.
